SMITHS FALLS, ON
The Perth Smiths Falls District Hospital COVID-19 Assessment Centre (CAC)
is situated at the Smiths Falls hospital site.
A referral is NO LONGER NEEDED to be seen at the CAC,
however an APPOINTMENT MUST BE MADE IN ADVANCE
by calling: 613-283-2330 ext. 1401
COVID-19 testing will only be performed on patients who meet the criteria for testing as determined by the Province.
Hours of operation:
Monday-Friday: 8 AM to 6 PM
Saturday: 10 AM to 4 PM
Sunday: 10 AM to 4 PM
