Health Promoter
Registered Practical Nurse
Part Time (0.8FTE) Temporary
As a Health Promoter at the Sharbot Lake Family Health Team (SLFHT) your role will involve working both independently and collaboratively with Physicians, Nurse Practitioners, Registered Nurses, and other allied health professionals (AHPs) to coordinate health promotion and deliver comprehensive patient care.
Minimum Qualifications, Education and Training
- College diploma in Registered Practical Nursing (RPN)
- Current certificate of Registration from the College of Nurses of Ontario (RPN)
Responsibilities
Program Support: Assist in the coordination and support of health promotion programs within the Family Health Team and Diabetic Education Program.
Triaging Phone Calls and Patients: Efficiently manage and triage calls and patient inquiries to ensure timely responses and appropriate routing to the healthcare team.
Bloodwork Requests: Conduct blood draws on request or through scheduled visits as requested by health care professionals.
Vitals and ECGs: Perform basic health assessments including blood pressure checks, vital signs, height, weight measurements, eye tests, and electrocardiograms (ECGs) as required.
Immunizations and Injections: Administer vaccinations and injections as prescribed by physicians and AHPs, adhering to medical protocols and ensuring patient safety.
Relaying Results to Patients: Communicate test results promptly and accurately to patients, providing explanations and ensuring patient comprehension and satisfaction.
Clinic Support: Ensure examination rooms, treatment areas, and other clinical spaces are kept clean and adequately stocked with necessary medical supplies, equipment, and patient education materials. Perform sterilization procedures for medical instruments and equipment according to established protocols and infection control guidelines.
Knowledge, Skills, and Abilities
- The position requires strong organizational skills, attention to detail, and the ability to prioritize tasks effectively in a dynamic healthcare environment.
- Experienced in performing phlebotomy, vitals, and administering injections and immunizations.
- Ability to work effectively both independently and within a multidisciplinary team and communicate clearly with patients, healthcare professionals and administrative staff.
- Commitment to community-based healthcare and knowledge of community-based resources.
- Excellent computer skills and experience using an electronic medical record system.
- At least 1-3 years of experience in primary care.
Position Details:
Title: Health Promoter
FTE: 0.8FTE (equivalent to 4 days per week)
Hours: 32 hours per week, with flexibility between Monday to Friday, 8:00am to 5:00pm
Contract: Temporary position until 31 March 2025, with the possibility of extension
Hourly: $29.00/hr
Benefits: 22.5% in lieu of benefits
