Part-time (.4FTE) Permanent

The Sharbot Lake Family Health Team is seeking a Registered Dietitian to work in partnership with the Registered Nurse CDE as part of the Diabetic Education Program. As a Dietitian at the Sharbot Lake Family Health Team (SLFHT) you will work both independently and in collaboration with physicians and other health care professionals to assess and treat health problems, with an emphasis on health promotion and diabetes management. 

Qualifications, Education and Training

  • Registered Dietitian with the College of Dietitians of Ontario
  • Certified Diabetes Educator or ability to complete CDE certification
  • Experience in Diabetic Education

Responsibilities

  • Work as part of an interdisciplinary team to provide overall health management in support of the Diabetic Education Program and primary care providers.
  • Utilize best practice strategies to provide diabetic, nutrition, and weight management education to patients, families, and care providers.
  • Consult with other team members to achieve preferable outcomes for patients.
  • Participate in diabetic care rounds and in-service education.
  • Review clinical information obtained through referrals, lab reports, and health records.
  • Support patients/families and primary care providers to develop, implement and revise customized self-management plans.
  • Serve as a community resource by providing information about diabetes, nutrition, health, and weight management.
  • Participate in regular program review, planning and evaluation initiatives with a continuous focus on quality improvement.
  • Perform administrative tasks as related to the position, including program planning, clinical documentation, statistics, and report preparation.

Knowledge, Skills, and Abilities

  • Knowledge of the role of the Dietitian within an interdisciplinary team.
  • Experience with managing and providing education to diabetic patients.
  • Commitment to community-based healthcare and knowledge of community-based resources.
  • Communicates effectively with patients, families, peers, and other health care professionals.
  • Demonstrated ability to share information and teach while treating patients.
  • Excellent organization, time management, communication, and interpersonal skills.
  • Proficiency in the use of TELUS PSS Suite an asset.
  • Excellent verbal and written communication skills

Position Details:

FTE: 0.4FTE

Hours: 2 days (16 hours) per week, with flexibility between Monday to Friday, 8:00am to 5:00pm

Contract: Permanent

Hourly: $35.65/hr

Benefits: 22.5% in lieu of benefits
