Part-time (0.4FTE) Permanent

As an Occupational Therapist at the Sharbot Lake Family Health Team (SLFHT) you will work both independently and in collaboration with physicians and other health care professionals to assess and treat health problems, with an emphasis on health promotion and chronic disease management. The OT role is to provide care and support to patients and their families, including cognitive assessments, physical evaluations, and assistance navigating community resources.

Qualifications, Education and Training

  • Registered with the College of Occupational Therapists of Ontario
  • Member of the Canadian Association of Occupational Therapists (CAOT) and/or Ontario Society of Occupational Therapist (OSOT) preferred
  • Experience in community and/or primary care medicine considered and asset

Responsibilities

  • Provide individual assessments and interventions focused on improving or maintaining function, promoting health, supporting caregivers, and system navigation.
  • Collaborate with members of the inter-professional primary care team to deliver comprehensive care and service coordination.
  • Collaborate with partner organizations as appropriate to address patient goals and promote smooth transitions of care.
  • Participate in regular program review, planning and evaluation initiatives with a continuous focus on quality improvement.
  • Consult with other team members to achieve preferable outcomes for patients.
  • Perform administrative tasks as related to the position, including program planning, clinical documentation, statistics, and report preparation.

Knowledge, Skills, and Abilities

  • Knowledge of the role of the Occupational Therapist within an interdisciplinary team.
  • Experience with managing and providing assessments and education to patients of all ages.
  • Commitment to community-based healthcare and knowledge of community-based resources.
  • Communicates effectively with patients, families, peers, and other health care professionals.
  • Demonstrated ability to share information and teach while treating patients.
  • Excellent organization, time management, communication, and interpersonal skills.
  • Proficiency in the use of TELUS PSS Suite an asset.
  • Excellent verbal and written communication skills

Job Location

The primary location of work is the Sharbot Lake Family Health Team, 1005 Medical Centre Road, Sharbot Lake ON.

Hours of Work

This is a part-time permanent position consisting of 16 hours per week

Compensation

Salary: approximately $29,650/year plus benefits
