The Sharbot Lake Family Health Team is currently accepting new patients.
Eligible patients must permanently reside within our catchment area. This area includes Central Frontenac, as well as portions of North and South Frontenac. Unfortunately, we do not have the capacity at present to accept patients who do not permanently reside within our catchment area.
To determine if you live within our catchment area, please complete the following form and an employee will reach out to you regarding your eligibility.
Please note that we are currently booking approximately 6 months in advance for New Patient Intake appointments.
If you are eligible to become a patient at the Sharbot Lake Family Health Team and have a current family physician, we recommend that you remain in their care until your new patient intake appointment takes place.
In the interim, should you have an acute medical issue arise, you may access our After-Hours Urgent Care clinics on Monday and Wednesday evenings from 5:00 PM to 8:00 PM. You do not require an appointment and do not need to be a rostered patient to access these clinics.
Please note, we are not able to accommodate chronic, ongoing issues at these after-hours Urgent Care Clinics. These clinics are strictly for urgent or emergent medical issues.
